Chiro vs Physio for Lower Back Pain: Which One Do You Actually Need?

It Is the Wrong Question If You Treat Them as Opposites

If your lower back has locked up, most people in Australia ask the same thing: chiro or physio?

They are not rivals. They are different tools. A chiropractor spends more time on joints, spinal movement, and nerve load. A physiotherapist spends more time on tissue loading, strength, and a structured exercise plan. Plenty of people need both. The useful question is which one you need first.

At Prime Wellness Chiropractic in Penshurst, we get this question every week from desk workers, gym-goers, and people who have already tried stretching and rest. This is the honest version.

What a Chiropractor Is Actually Doing

If a lumbar joint is restricted, if the pelvis is not sharing load, or if a disc is irritating a nerve, the pain often shows up as stiffness, a sharp catch, or a line down the leg.

The work is to restore that movement, take pressure off the nerve, and change how you sit, hinge, and lift so the same pattern does not reload the next day. We treat on the first visit. You should leave knowing what is driving the pain, not just a label of "back pain."

What a Physiotherapist Is Actually Doing

Physio is built around loading tissue in a planned way. If a muscle, tendon, or movement pattern is weak or poorly timed, a physio will usually give you a graded exercise program, hands-on work where it helps, and a return-to-work or return-to-sport plan.

That is the right first step after many surgeries, for tendon problems, or when you already know the joint is moving and the missing piece is strength and control. It is also useful later in care, once the back is moving again and you need the hips and core to keep it that way.

A good physio is not "just stretches." A good chiropractor is not "just cracking." Both of those stereotypes waste people's time.

When to Start With a Chiropractor

Start here if most of this sounds like you:

  • The back locks, catches, or feels jammed after sitting or a long drive
  • Pain eases when you walk and comes back when you sit
  • You get pins and needles, a sharp line down the leg, or the hamstring always feels tight no matter how much you stretch it
  • A deadlift, squat, or awkward lift lit it up, and the joint still will not move normally
  • You have already done weeks of exercise and the same segment still will not settle

When Physio Is the Better First Step

Start with a physiotherapist if:

  • You are following a post-operative protocol and need a graded loading plan
  • The main problem is a tendon, a sprain that needs a strength progression, or a clear sports-rehab program
  • The joints already move well and what you lack is capacity, control, or a return-to-sport ladder
  • Another practitioner has already cleared the spine and told you the next step is loading

If that is you, physio is not a lesser option. It is the right tool for that job.

When You Might Need Both

This is common. The joint needs to move, and the hips and core need to share the load afterwards. People who only get adjusted and never rebuild control often flare again. People who only strengthen on a restricted spine often keep hitting the same wall.

A simple sequence we use a lot: get the back moving, change how you sit and hinge, then build the strength that keeps it. Our lower back stretches are a useful adjunct. They are not a substitute for either profession.

How to Decide This Week

Ask yourself one question: does this feel stuck, or does this feel weak?

Stuck, catching, nerve-like, worse with sitting: start with a chiropractor. Weak, deconditioned, post-op, or clearly a loading problem: start with a physio. Unsure: come in and get assessed. We would rather tell you if physio is the better first step than keep you in the wrong plan.
